Insurance Payers and Billing:
The Centers accepts Medicare, Florida Medicaid, and many major commercial health plans. It is your responsibility to update The Centers with the most current insurance information by providing a copy of your insurance card. As a courtesy The Centers will bill your insurance plan for services rendered.
All co-pays, co-insurance and deductibles are your responsibility. When applicable, co-pays and co-insurance are expected at the time of service.
Please contact the Billing office, 1-352-291-5555 option 3, for any questions or concerns that you may have regarding your account. All client responsibility questions regarding co-pays, co-insurance, and deductibles should be addressed with your insurance carrier.
Financial assistance may be available to clients who have limited or no resources. The Centers utilizes a sliding fee scale that is based on income and family size. If a financial assessment reflects 200% or below of The Federal Poverty Guidelines for your household income/size, you may qualify.
Federal Poverty Guidelines can be found at: https://aspe.hhs.gov/poverty-guidelines
To apply for financial assistance please print and complete Financial Assessment and bring to your next appointment. The Centers may request supporting documentation such as pay stubs, tax return, employment verification or other governmental agency verification as to your status.
